Media Release and Medical Waivers

Media Release

The Educational Garden Project, LLC may use photos or videos of child(ren) participating in order to promote our mission or our partner organization’s missions in order to increase public awareness of our programs through digital media and printed media. 

I, the parent or guardian hereby give The Educational Garden Project, LLC’s employees and authorized media organizations permission to print, photograph, and record my child(ren) for use in audio, video, film, or any electronic, digital, and printed media. 

Emergency Medical Treatment Waiver  

Should myself or my child be injured during any activity, I hereby authorize any medical care that is deemed in my best interest from trained employees. 

I, the parent or guardian hereby give The Educational Garden Project, LLC permission to secure emergency transportation for me or my child in the event of an illness or injury which requires emergency medical treatment. The emergency transportation will determine the facility to which myself or my child will be transported.

Allergy Agreement

The Educational Garden Project, LLC can not guarantee an allergen-free environment. If your child(ren) has any food allergies please make sure to be completely transparent with staff members about allergies. We will seek to eliminate the risk of exposure to your child(ren) but understand there is always a risk of contamination. 

I, the parent or guardian hereby release the Educational Garden Project, LLC and its staff from responsibility if my child has an allergic reaction while attending Garden Camp.

COVID-19

The Educational Garden Project, LLC has put in place preventative measures to reduce the spread of COVID-19; however, The Educational Garden Project, LLC cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending any programs at The Educational Garden Project, LLC could increase your child(ren)’s risk of contracting COVID-19, and such exposure or infection may result in personal injury, illness, permanent disability, and death.

I, the parent or guardian hereby assume responsibility if anyone in my household contracts COVID-19 and certify that I will not bring them to camp if anyone in my household has any symptoms of illness including, but not limited to, fever, cough, shortness of breath, or body aches.

Release, Indemnity and Assumption of Risk

In consideration of my child’s participation in The Educational Garden Project, LLC’s programs and activities (the “Program”), I, on behalf of myself, my child, and our personal representatives, heirs and assigns, fully and forever release, discharge, indemnify and hold harmless The Educational Garden Project, LLC, its owners, officers, employees, independent contractors, volunteers, agents, successors and assigns (“Released Parties”), from any and all claims for property damage, personal injury or death that may arise from the participation of my minor child in the Program, including, without limitation, any and all claims, causes of action, liabilities, damages or costs of any nature whatsoever arising from the negligence or carelessness of the Released Parties in connection with my child’s participation in the Program.

I acknowledge that the Program includes children having access to tools, glass, and other sharp or heavy objects. Additionally, children may be exposed to bees, snakes, bears, dogs, poison ivy, stinging nettles, brambles, thorns, and other items familiar to our Program. I understand that there is a possibility that such activities could result in personal injury and/or property damage, whether foreseen or unforeseen, to my child. I do hereby freely and voluntarily choose to have my child participate in the Program and hereby expressly assume the risk, including financial responsibility, of any and all liabilities or injuries to my child that may occur during or as a result of the Program.

Prior to signing this Release, Indemnity and Assumption of the Risk, I acknowledge that I have read it, understand it, and have had an opportunity to ask questions about it.
